My psychiatrist was the person who recommended me to file disability. I had a few physical things but it was mostly mental. I applied the first time and got denied. So I went to this form to get help. They would not even help me get my records. I have met other people who have said the same thing. Basically, after you do all of the work the might pick up your case and represent you in court. I called them and told them that my physical doctor would not give me my records and they did nothing.
