Daniel was born and raised in Marietta, Georgia. He graduated from the University of Texas at El Paso with a Bachelor of Arts in Political Science. He earned his Juris Doctorate from the University of Texas, and he holds a Master of Healthcare Administration from Seton Hall University and a Master of Business Administration, with a concentration in International Business, from the University of Texas at El Paso. Daniel holds a double certification in Healthcare Compliance and as a Compliance and Ethics Professional. Daniel manages the Law Office of Daniel R. Crumby, LLC, where he focuses on complex litigation and business law including white-collar defense; defense of corporate, and individual clients in state and federal courts around the country; medical malpractice; nursing home and elder abuse; healthcare compliance and investigations; personal injury, and criminal law. Prior to starting his practice, Daniel worked at a mid-sized law firm in Atlanta, and he specialized in criminal and civil healthcare-related white-collar cases, medical malpractice, healthcare compliance, internal investigations, and defending insurance companies and their insurers in complex cases, including lawsuits filed against skilled nursing facilities and assisted living facilities. Immediately prior to working at a civil defense firm, Daniel defended the United States and its agencies in federal and state courts, and he served as a federal prosecutor in the Southern District of Georgia and the Western District of Texas. As a federal prosecutor, Daniel directed and prosecuted numerous cases including healthcare fraud, bank fraud, procurement fraud, mortgage fraud, wire fraud, theft of government funds, drugs and alien smuggling, child exploitation, human and sex trafficking, and other criminal violations. In 2014, Daniel received a Commendation from the Federal Bureau of Investigation Director, James B. Comey. Daniel previously served in the United States Army as military police and as a Judge Advocate General (“JAG”) officer in a variety of assignments before resigning his commission as a Captain. The Army twice awarded Daniel the Meritorious Service Medal, Army Commendation Medals, Army Achievement Medals, and the Fort Bliss Ambassador Award, among others, for his military service. Daniel is a member of the State Bar of Georgia, State Bar of Texas, the United States Court of Appeals for the Fifth Circuit, and the United States District Courts for the Northern District of Georgia and the Western District of Texas.

Born and raised in Northern California, Mona Deldar obtained her Bachelor’s degree with Honors from the University of California, Berkeley. After an internship at a personal injury law firm, Ms. Deldar was inspired to apply to law school. She attended the University of San Francisco School of Law where she was a member of the Maritime Law Journal. She received her Juris Doctorate degree in 2003. Ms. Deldar’s practice is dedicated to representing injured plaintiffs in a broad range of catastrophic personal injury and wrongful death cases. Ashley Ameika, Esq. received her Bachelor's Degree from the University of Notre Dame where she was consistently on Dean's List and graduated cum laude. She then went on to receive her Juris Doctor from Thomas Cooley School of Law, where she was on the Dean's List. While in law school she participated in the 60+ Elder Law Clinic and she concentrated in the field of Litigation. She is licensed and is a member in good standing in South Carolina in State and Federal Court. She practices in the areas of serious personal injury, criminal defense, and domestic litigation.

Yandy Reyes is an American born, first-in-family to graduate college and law school, first-in-family to serve in the U.S. Army as a decorated commissioned officer, and first-in-family to become a licensed attorney. At the university level, Attorney Reyes excelled because of his insatiable curiosity to learn more, to do his best, and to strive for excellence always, without compromise, and no shortcuts. While in John Jay College, CUNY, Attorney Reyes maintained a job, a 3.93 GPA (honors), and earned a Black Belt in Goyu Karate and a Brown Belt in Judo. While in law school, Attorney Reyes earned awards for top marks, inter alias, in “Elder Law” and “Legal Profession,” the former becoming his preferred area of law, and the latter being the code of ethics that governs attorneys. Adam joined Fitzpatrick & Associates in 2017. He concentrates his practice in Worker’s Compensation, Social Security Disability, Personal Injury, Employment law, and Automobile accidents. Adam attended New England School of Law │ Boston, and received his Juris Doctor Magna Cum Laude in 2016 . While in Law School Adam was on the Dean’s list all semesters, received the New England Scholar Award from 2013-2015, was awarded the Trustee’s Academic Scholarship from 2014-2016. Adam served as a Judicial Intern for the Honorable Judge Raymond Brassard in Superior Court and he served as a student Attorney for the New England Law│Boston Clinical law Office representing clients in Social Security Disability, Family Law, and Elder Planning. Justine grew up in the small farming town of Woodland, California. She stayed in the area for college, graduating from the University of California, Davis with a B.A. in Anthropology in 2013. In addition to scholarships and grants, Justine funded her undergraduate education with her job as a student bus driver. After graduating, Justine attended the University of Kansas School of Law on a full-tuition scholarship. While in law school, Justine excelled academically – earning election into the Order of the Coif, the 2016-2017 Robert E. Edmunds Prize in Corporation and Securities Law, the CALI Award in Secured Transactions for Spring 2017, and the CALI Award in Trusts and Estates for Spring 2017. Justine also participated in the 2017 Transactional LawMeet, in which her team won the Midwest Regionals, advanced to the National Finals, and won the National Best Draft Award. She also served as a Publications Editor for the Kansas Journal of Law & Public Policy and as a research assistant for J.B. Smith Distinguished Professor of Constitutional Law, Richard E. Levy. In addition to her academic pursuits, Justine worked as a judicial extern for the Honorable James. P. O’Hara, United States District Court, District of Kansas, and as an elder law intern with Kansas Legal Services. She also worked as a summer law clerk for Hill’s Pet Nutrition, Inc. and a general practice law firm in Lawrence, Kansas. Justine moved to the Tri-Cities shortly after law school graduation, joining her husband who works for the railroad. She joined Leavy Schultz Davis, P.S. in the fall of 2017 as a litigation associate after successfully passing the Washington State Bar Exam.
