How Do I File a State Administrative Complaint?
The complaint must allege a violation that occurred within one year prior to its submission, and it must be submitted in written form and signed. The following must be included in the complaint:

The school district, department of education, or other public agency or the state statutes and regulations that did not uphold federal (IDEA) or state special education laws

The facts and information that the complaint is based on

The signature and information for the individual or organization who filed the complaint
If the complaint involved a certain child, it must include:

The child’s name and address

The name of the child’s school
-
School and contact information for the child if they are homeless

A description of the child’s problem (including related facts)

A suggested solution to the problem based on the available information at the time the complaint is filed
The individual or organization submitting the complaint must send a copy of it to the school district against which the complaint is filed right when they submit it to the CSDE.
