All local or regional boards of education, as well as any institution or facility operating under contract with them, must:

Document every instance of physical restraint or seclusion used on a student.

Detail the nature of the emergency that required the use of such restraint or seclusion.

Include this information in an annual report on the use of restraint and seclusion on students.
In Connecticut, the State Department of Education is required to gather data from these boards of education and associated institutions. The required data for students at risk or identified as special education includes:

All instances of emergency use of restraint and seclusion

The status of the student (whether they are eligible for special education or are in the referral process)

The nature of the emergency that necessitated the use of restraint or seclusion

All instances of physical injury resulting from restraint or seclusion, including serious injuries that require attention beyond basic first aid
